Seek Optimal Health Notifies Patients of Data Security Incident

Seek Optimal Health has become aware of an incident that exposed a small number of patients’ email addresses. We take patient privacy very seriously and we have taken steps to notify any patients who may have been affected by this incident, including sending letters to anyone whose email address might have been exposed.

On January 3, 2019, while emailing some individuals to inform them of some promotions we were offering, we inadvertently copied other addresses to the email. Therefore, it is possible that these individuals’ email addresses were viewed by the other recipients. This matter only impacted the email addresses, and other information, including their Social Security Number and medical information, were not disclosed. At this time we have no reason to believe that these email addresses have been misused by any of the recipients.

We have taken steps to prevent this from happening in the future, including reviewing our policies and procedures and training our staff to be aware of these policies with respect to patient communications, including any promotional or informational emails. Notification letters were mailed on March 4, 2019, including additional information about what happened and a toll-free number where the impacted individuals can learn more about the incident.
