Other Fees:

Remote New Patient Visits ($350): To optimize our availability and timeliness in getting new patients seen, we offer the option of scheduling a 60 minute video visit to review history and make initial recommendations. We strongly recommend that this is followed soon after with a 30 minute in-person visit so that we can do a physical exam and osteopathic structural exam and any appropriate osteopathic manipulative treatment.

Appointment cancellation: Appointments cancelled with less than 24 hours notice may be subject to a fee equivalent to half the cost of the appointment.

Form Completion: $250/hour at 15 minute increments 

This applies to forms for work, insurance, disability, or any other form that requires physician time outside of their normal scope as determined by the physicians.

Records Request: $50/hour
If records are required for insurance or disability reasons, a processing fee of $50/hour applies for administrative time.​

NOTE: There is no fee for communicating with our physicians via messages in the patient portal. This communication, however, is not intended to take the place of regular visits.
