Missed Appointments/Late Cancellation

Appointments cancelled without 24 hours notice may be subject to a cancellation fee equivalent to the cost of the missed visit.

Form Completion: $250/hour at 15 minute increments 

This applies to forms for work, insurance, disability, or any other form that requires physician time outside of their normal scope as determined by the physicians.

Records Request: $50/hour

If records are required for insurance or disability reasons, a processing fee of $50/hour applies for administrative time.​

NOTE: At this time, there is no fee for communicating with our physicians via messages in the patient portal. Use of messaging is meant to facilitate communication, but not intended to take the place of regular visits.
