Other Fees:

Appointment cancellation: Appointments cancelled with less than 24 hours notice will be subject to a fee of half the appointment cost as above.

Form Completion: $250/hour at 15 minute increments 

This applies to forms for work, insurance, disability, or any other form that requires physician time outside of their normal scope as determined by the physicians.

Records Request: $50/hour
If records are required for insurance or disability reasons, a processing fee of $50/hour applies for administrative time.​

NOTE: There is no fee for communicating with our physicians via messages in the patient portal. This communication, however, is not intended to take the place of regular visits.
